Completed W-4

Completed I-9

Appalachian Staffing Basic Information Sheet

Appalachian Staffing Employment Agreement

Photos of the following:

EITHER JUST a passport

OR

A Driver's License AND one of the following (not both):

Social Security Card

Birth Certificate

Additional details about what forms of identification count can be found on the second page of the I-9 form.

Please complete this BEFORE your first scheduled work shift with us.

The W-4, I-9, and Basic Information Sheet are editable PDFs that can be filled out digitally.

The Employment Agreement will have to be printed, signed, and scanned.

If you do not have access to a printer, please just fill these out digitally and submit them.
