Please complete this BEFORE your first scheduled work shift with us.

The W-4, I-9, and Basic Information Sheet are editable PDFs that can be filled out digitally.

The W-4 and I-9 require signatures, so please fill them out digitally, print and sign them, then scan them.

The Employment Agreement will have to be printed, signed, and scanned.

If you do not have access to a printer, please just fill these out digitally and submit them. We will have them printed and ready to be signed by you before your first work call begins.
